  • To share information: You may need to give your health info to a new doctor, a specialist, family, or even a school.
  • To make updates: If anything in your record is incorrect, you can request a correction to ensure you and your care team are working with the right information.

Every health care provider you’ve visited keeps their own records about your care. That means your full health history is usually spread across multiple places. To get a complete picture, you’ll need to request your records from each provider you’ve seen.

Please fill out the Authorization to Release Information form (found here) in its entirety and submit to HIM@daphealth.org.

Yes, but you will need to fill out our Authorization for Third Party Health Information Access to designate spouse to your medical records.
