Why Might I Request My Full Medical Records?
There are a few key reasons:
Who Has My Medical Records?
Every health care provider you’ve visited keeps their own records about your care. That means your full health history is usually spread across multiple places. To get a complete picture, you’ll need to request your records from each provider you’ve seen.
What do I need to provide to request my records?
Please fill out the Authorization to Release Information form (found here) in its entirety and submit to HIM@daphealth.org.
Can I make an expedited (rush) request?
Yes, you can. Please call 760.323.9255, ext. 771.
Will you mail the records, or will I have to pick them up?
You will find alternative methods of delivery on the Authorization to Release Information form.
Can my spouse (or anyone else) request and receive copies of my medical record?
Yes, but you will need to fill out our Authorization for Third Party Health Information Access to designate spouse to your medical records.
What are DAP Health’s mobile services?
Our mobile services bring health care directly to the community — offering primary care, sexual health screenings, behavioral health support, vaccinations, and more from fully equipped mobile units. We go where care is needed most, reducing barriers like transportation, stigma, and cost.
What does home care include?
Our home caregivers provide a range of services, from medical assistance and daily living support to light housekeeping and companionship, helping clients stay independent at home.
Who can use mobile services?
Everyone is welcome. We serve people of all ages, backgrounds, identities, and insurance statuses — whether you’re housed or unhoused, insured or uninsured, a first-time visitor or a returning patient.
