Maintaining physical and mental well-being is crucial for a healthy workforce. DAP Health goes beyond just offering comprehensive health benefits like insurance and flexible spending accounts. We help employees enhance their physical and mental well-being, prevent health problems, engage in health-promoting behaviors, and find assistance and support during times of need.

Health Insurance 

We partner with Cigna to offer various medical insurance plans, including a PPO (preferred provider organization) plan with a $3,000 or $1,000 deductible, or a HDHP (high-deductible health plan) with a $3,300 deductible and an HSA (health savings account).
Dental coverage is offered through Cigna as a DHMO (dental health maintenance organization) plan or a dental PPO (preferred provider organization) plan.
Vision coverage is also offered by Cigna as a PPO (preferred provider organization) plan, covering exams, lenses, frames, and contacts.

We offer both a health care FSA (flexible spending account) or a limited-purpose FSA. Eligible employees can open a Lively HSA (health savings account) if enrolled in the HDHP. DAP Health matches up to $700 annually.

  • HSA: $4,300 for individual coverage and $8,550 for family coverage. Additionally, those aged 55 and older are eligible to make a catch-up contribution of $1,000.
  • FSA: The maximum salary deferral is $3,300, with a rollover maximum of $660.
Our STD (short-term disability) coverage provides a benefit equal to 66.67% of your earnings, up to $1,850 per week for a period up to 17 weeks. Our LTD (long-term disability) coverage can replace 66.67% of your earnings, up to a maximum of $8,000 per month if your disability extends beyond 180 days.
Our voluntary benefits include accident, critical illness, and hospital protection insurance, all available at the employer’s cost.

Life and AD&D

Eligible employees are provided with a basic life and accidental death and dismemberment (AD&D) insurance of $25,000 through Mutual of Omaha. Employees may also purchase supplemental life insurance for themselves, their spouse/domestic partner, and/or children.
Employees have access to free and confidential resources, information, and counseling for matters like legal, financial, and mental health counseling.
We participate in the Fidelity Retirement Plan, which includes a 401(k) retirement plan and a 403(b) Roth plan. After two months of service, new employees are automatically enrolled in the 401(k) plan at a 3% contribution rate. We match up to 7% of employee contributions.
Our 457(b) deferred savings plan is a great way for employees to enhance their retirement savings. By taking advantage of this plan, they can increase their contributions toward their retirement goals and enjoy tax-deferred growth on their savings.
Cigna members can take advantage of DAP Health’s extensive health care resources, including MDLive, Meru Health, Talkspace, Ginger, and Happify. In addition, members can access healthy rewards discounts, such as a $25/month nationwide gym membership discount, Daily Burn virtual fitness classes, Fitbit discounts, and more.
IdentityForce, available through Cigna, offers identity theft protection that proactively monitors, alerts, and assists in the resolution of any identity theft compromises. It covers the employee and any children (up to age 18) living in the household.

Holidays  

  • DAP Health provides up to 12 paid holidays each calendar year (depending on when the holidays fall). All full-time employees are eligible for paid holidays and will receive a list of the holidays the organization will observe each year.
  • Employees will receive a full day (eight hours) of pay for each holiday if they work full-time. If they are a modified full-time employee, they are paid for a holiday only if it falls on a day they are normally scheduled to work, and they are paid holiday pay for the number of hours they would have normally been scheduled to work (ex: if they were scheduled to work six hours, they would receive six hours of holiday pay).

Paid sick leave  

DAP Health’s paid sick leave policy is designed to provide financial protection if an employee is unable to work because of illness (theirs or a family member’s), injury, or other medical reasons (such as doctor’s appointments). Paid sick leave can also be used if an employee is a victim of domestic violence, sexual assault, or stalking.

To ensure the wraparound wellness of its employees, DAP Health created self-care time off. This benefit gives employees one paid day off (a half day for part-time employees) every quarter to enjoy as they choose.

Vacation 

  • DAP Health recognizes the need for employees to take vacations or extra days off to have a more balanced work/life experience. We offer all regular full-time employees paid vacation time.  
  • All full-time and modified full-time employees are eligible to earn paid vacation time on a pro rata basis in accordance with this policy’s terms. The number of days an employee earns depends on their length of employment, their position, and the number of hours they work.  
  • An employee begins to accrue vacation time on their first day of employment and can use it as it is accrued.
