Jordan Michael Hall

Board Member

Native Southern Californian Jordan Hall has over 19 years of expertise in population
health management, accountable care delivery models, and accountable care
organizations (ACOs) — not to mention a diverse background in both commercial and
government alternative payment frameworks. He offers leadership and direction to
health care organizations and other value-driven health care entities, ensuring
regulatory compliance and quality standards are met. His strengths encompass team
building, risk stratification, coding and quality assurance, care management, prompt
problem-solving, precise data analysis, and project management.


With a wealth of experience in process enhancement, Lean methodologies, and data
governance, Hall has professional know-how in both internal and external data analytics
solutions. He’s held executive roles in population health management, and has been
involved in ambulatory care coordination and redesign innovation projects at UCLA
Health, Apollo Medical Holdings, Aegis Medical Group, Aledade, and numerous other
value-based organizations as a principal consultant.


At UCLA Health, he takes great pride in his role in developing and leading a multisite
care coordination and medical home model, focusing on primary care redesign,
increasing the number of patients under population management, expanding primary
care capabilities, and fostering collaboration and innovation.


A current resident of Boise, Idaho, Hall is a dedicated member and advocate for the
LGBTQIA+ community. He earned a Bachelor of Science in nursing from the Uniformed
Services University of the Health Sciences/Johns Hopkins University, and his MSN and
DNP from California State University, Los Angeles.


He has served with honor at Walter Reed Army Medical Center Military Advanced
Training Center (providing amputee and rehab care), Naval Hospital Camp Pendleton,
and Wounded Warrior Regiment, and served the veterans community at VA facilities in
Baltimore, Maryland, and at the VA Central Office (VACO) in Washington, D.C.
Hall is a DAP Health volunteer and a proud member of the Partners for Life major donor
program.
