Our Social Services team is dedicated to helping you navigate and access the resources they need to maintain stability so they can focus on their health.

Our case managers work closely with you to evaluate eligibility for available programs, ensuring you receive comprehensive support tailored to your unique circumstances. Whether it’s helping you find reliable transportation to medical appointments, securing emergency or permanent housing, or connecting you to vital food assistance programs, our goal is to break down the obstacles that prevent access to care.

Transportation Assistance
Transportation should never be a barrier to receiving health care. We help clients access transportation assistance programs, ensuring they can attend medical appointments without worry.
Food Assistance

Our home caregivers bring essential support services directly to clients, offering medical assistance, companionship, and housekeeping services, allowing individuals to remain independent and safe in their homes.
