Medical Records

How to request your medical records

You can request your records electronically or by submitting a completed authorization form. Here are your options:

MyChart

You can access your medical records anytime through MyChart, DAP Health’s secure patient portal. MyChart allows you to view and download key parts of your medical history, including visit summaries, test results, and immunization records.

If you cannot find the records you need, you can also submit a request directly through MyChart. Once your request is processed, the records will be added to your MyChart account, where you can easily view, download, or print them from your device.

Other ways to request your records

If you prefer not to use MyChart, there are several other ways to request your medical records from DAP Health. You will need to complete an Authorization for Release of Information form, which can be submitted using any of the methods below.
Once your request is received and processed, your records will be sent securely through your preferred method of delivery. Please allow time for processing, and ensure your form is filled out completely to avoid delays.

Health Information Exchange (HIE) & OCHIN

DAP Health participates in an organized health care arrangement with other providers through OCHIN, a health information network. As our technology partner, OCHIN supports electronic health records, care coordination, and quality improvement efforts across participating organizations.

Your health information may be shared with other OCHIN participants or through a health information exchange only when necessary for treatment or health care operations, such as improving care coordination or referral management.

This information may include your past, present, or future medical details, and will only be shared in accordance with federal Privacy Rules and applicable laws. You may withdraw your consent at any time, though prior disclosures may still apply.

EPIC Care Everywhere / HIE Sharing

As a DAP Health patient, your information is automatically included in our Health Information Exchange (HIE), unless you choose to opt out.

To opt out, email: opt-out@daphealth.org. You will be removed from all HIE portals.

Please note that opting out may affect your care:

  • Other doctors may not have access to your complete medical history.
  • You may need to provide information manually for each provider.
  • Treatment or diagnosis may be delayed without shared access to records.

Have a question about medical records?

If you or your external physician have questions about requesting medical records, please contact DAP Health’s Health Information Management Department Monday to Friday, 8 a.m. to 4 p.m., excluding weekends and holidays. Due to the high volume of calls, email or fax method is highly encouraged.
