MyChart is a secure online health portal developed by Epic Systems, one of the largest providers of electronic health records (EHR) in the United States.

It gives patients direct access to their personal health information, making it easier to manage care from anywhere.

It’s typically offered by hospitals, clinics, and health systems that use Epic’s EHR software (as DAP Health does). Patients access it via web browser or the MyChart mobile app (iOS and Android).

There are a few ways to register:

  • Option A: With an Activation Code
    • You may receive a code during a DAP Health visit, on your billing statement, or via email/text.
    • Go to DAP Health’s MyChart landing page and select “Sign Up Now.”
    • Enter your activation code, ZIP code, and birth date.
    • Set your username, password, and security questions.
  • Option B: Without an Activation Code
    • Choose “Sign Up Online” or “Request Access” from the login page.
    • You’ll need to verify your identity (usually with name, DOB, and last four digits of your SSN).
    • Some systems may use secure third-party ID verification.

You can also download the MyChart app from the App Store or Google Play, and follow the same sign-up steps.

  • On the login page, click “Forgot password?”
  • Enter your username and date of birth.
  • You’ll receive a verification code via email or text.
  • Follow the instructions to create a new password.

If you’re locked out, contact our MyChart help desk for assistance (phone/email listed on the login screen).
