Proxy information for teens

In California, when a child turns 12, parents will have limited access to their child’s MyChart account because of privacy laws. These laws give teens (ages 12 to 17) the right to keep some medical information private.
After this permissions update, teens can talk with their care team about their privacy at their next in-person medical visit. If a teen wants to give their parent or guardian access to their MyChart account, the teen must sign a special form called the DAP Health Authorization for Third-Party Health Information Access. Teens can ask their care team to stop parent/guardian access to their MyChart account at any time.

What is MyChart Proxy Access?
MyChart Proxy Access means that someone else, like a parent, can see and manage a patient’s MyChart account and medical records.
For Parents of Kids Ages 12-17

When your child turns 12, your MyChart access will be changed to “Proxy-Teen Access,” meaning you’ll have limited access because of California’s privacy laws. You can still contact your child’s care team to make appointments by calling 760.323.9255.
If your child has a disability or other special needs, please contact their care team to talk about exceptions. Teens must sign the DAP Health authorization form (Third-Party Medical Information Access) to give parents any type of proxy access. Teens can set up their own MyChart account by talking to their care team at their next visit. They can also ask to change or stop parent access to MyChart at any time.

How to File a Complaint (Grievance)

If you have any concerns, email us at patientrelations@daphealth.org. Privacy laws limit what we can share about your teen’s health, but DAP Health must tell you if we think your child is in danger. For more information on California’s privacy laws, please click here.
If you have questions or need help with MyChart proxy access, contact your child’s care team.
If you’re a teen in California, you have the right to talk about your health and make some medical decisions with your care team by yourself.
If you are under 18, your care team cannot tell your parent or guardian if you get these services:

  • Birth control
  • Emergency contraception
  • Pregnancy testing, prenatal care, or abortion
  • Testing and treatment for STIs/STDs
  • HIV/AIDS testing
  • Mental health services
How Do I See My Medical Records or Test Results?
When you turn 12, you can make your own MyChart account with your own username and password. MyChart lets you see your medical information, like test results and prescriptions. To get help setting up your account, visit a DAP Health clinic and bring a school or photo ID. We can help you download the MyChart app, send messages to your care team, and request appointments.
What If I Don’t Want My Parent/Guardian to See My Medical Records?
If your parent or guardian wants to see your MyChart records and you agree, you both need to visit a DAP Health clinic to sign the DAP Health Authorization for Third-Party Health Information Access form. You can decide not to share your records with your parent/guardian, and you can change your mind later if you want.
What If Something Happens With My Health Records That I Don’t Like?

We are here to help. You can file a complaint (called a grievance) with DAP Health if there’s a problem with your MyChart account. You won’t get in trouble for filing a complaint. Grievance forms are available at all DAP Health clinics. You can also ask for help at the front desk or call 760.323.9255.
