DAP Health releases 2024 Impact Report and Future Forward: DAP Health Strategic Plan 2025-2028.
At DAP Health, transparency is more than a value — it’s a promise to every member of the diverse communities we serve, each of whom makes the fulfillment of our mission possible. In that spirit, I am very proud to share two vital documents: our 2024 Impact Report, as well as our recently developed Future Forward: DAP Health Strategic Plan 2025-2028.
The 2024 Impact Report reflects a transformative year. Following DAP Health’s acquisition of Borrego Health on August 1, 2023, our nonprofit grew from a two-location clinic to a network of more than 25 community health centers throughout Southern California. The report highlights key accomplishments in patient care, community outreach, and health equity. From primary care, OB-GYN, pediatrics, dentistry, and behavioral health to LGBTQ+ specialty care, social services, harm reduction efforts, sexual wellness services, and STI/HIV testing, prevention, and treatment, the numbers tell a story of impact, growth, and continued passion and compassion.
But understanding where we’ve been is only half the picture. Future Forward: DAP Health Strategic Plan 2025-2028 outlines where we’re going. Shaped by months of collaboration and input from team members, board leaders, and varied community voices, the plan sets ambitious but achievable goals divided into four pillars: the advancement of health equity, care for employees, the patient and client experience, and resources and partnerships. Together, these pillars serve as our compass from 2025 through 2028.
Why are we sharing this now? Because openness builds trust — and trust fuels change. Whether you’re a patient, a team member, a donor, a volunteer, a policymaker, or a neighbor, you deserve to know where we stand and where we’re headed.
These are your community health centers. This is your impact. This is your future. Your support, trust, and engagement make our success and progress possible. I invite you to please look through these windows into our recent past, and our near future, today — because where we’ve been, we’ve been together. And we’re going, we’re going together, too.
